Friendly Fire Incident with USS Guardfish
Proceeding independently from Guam to the Philippines, Extractor was mistakenly identified by USS Guardfish (SS-217) which fired a torpedo striking Extractor’s starboard side. She capsized and sank within 5 minutes of the accident. ... Six lives were lost, but the remainder of her crew was rescued by Guardfish.
